Below is a copy of the participant waiver that you will be required to sign upon registration online:
2024 Victoria Junior Field Hockey Association Waiver
I hereby consent to the attendance and participation of a minor for whom I am legally responsible, (the Participant) in the 2024 Victoria Junior Field Hockey Spring League (the League) and, I agree to be bound by the terms of this Agreement. I understand and agree that the Participant's participation in the games, practices and related activities of the Victoria Junior Field Hockey Association (VJFHA) will not be permitted without execution of this Agreement, and is conditional upon my execution of this Agreement.
I hereby acknowledge and agree that:
- I am 19 years of age or older.
- The Participant is a minor for whom I am legally responsible.
- I understand that I am solely responsible for the personal safety of the Participant and that all rules for participation must be followed, including, but not limited to, the following:
- Shin guards and mouth guards must be worn during participation in games and practices.
- Participation will not be permitted without the use of this equipment.
- Mouth guards are required for insurance purposes and failure to wear a mouth guard may void coverage for mouth injuries sustained.
- The coaches and managers are unpaid volunteers and are not expected to supervise Participants outside of practice and game times. Participants who are repeatedly left unattended outside of these times may be asked to leave the program.
- From time to time, photos may be taken of league activities for promotional use. Names will not be used with these photos.
I understand, acknowledge and accept there are risks associated with participating in the sport of field hockey, including the possibility of injury. I agree to assume completely those risks of whatever nature and however caused for myself and on behalf of the Participant.
I waive any and all claims I may now and in the future may have against, and release and hold harmless from all liability, VJFHA, its board, agents, directors, officers, members, employees, representatives and volunteers, as well as any other corporation or person directly or indirectly involved in the League and its associated activities for any loss, cost, expense, damage or injury to person or property that results from the Participant's participation in the League or League related activities.
I have read and understand this Agreement and I am aware that by agreeing, I am giving up my right to sue and waiving certain legal rights. I sign this document voluntarily and without inducement.
